Thanks to our partnership with the Veterans Choice program, if a veteran’s local VA provider cannot serve them due to lack of availability or other factors, they have the choice to receive care at a Trilogy senior living community. Our campuses across Kentucky, Indiana, Ohio and Michigan provide a full continuum of care and support services, including:

What is the Choice Program

If you are already enrolled in VA health care, the Choice Program allows you to receive health care within your community. Using this program does NOT impact your existing VA health care, or any other VA benefit. Please visit the Veteran Choice Program website for more information.

Determining Eligibility

You must have been enrolled in VA health care on or before August 1, 2014, or you are eligible to enroll as a recently discharged combat Veteran. Additionally, you must meet at least one of the following criteria:

  • You are told by your local VA medical facility that you will need to wait more than 30 days for an appointment from the date clinically determined by your physician.
  • Your home is more than 40 miles driving distance from the closest VA medical facility.
  • You face an unusual or excessive burden in traveling to the closest VA medical facility based on geographic challenges, environmental factors, or a medical condition.

Scheduling an Appointment

If you are a Distance-eligible Veteran call 1-866-606-8198 to confirm eligibility and ensure the care needed is covered by VA. When you call, you will be asked for:

  • Your first and last name
  • Your full address
  • The name of your preferred community physician. Unfortunately, not all physicians will be eligible to participate. If your preferred physician is not available, representatives can recommend other physicians in your area.

If you are a Wait List-eligible Veteran, you will receive a phone call from a VA partner (a non-VA phone number) to help set up your appointment. When you receive this call you will be asked for:

  • Your first and last name
  • Your full address
  • Your date of birth
  • The last 4 digits of your social security number

Out-of-Pocket Expenses

If you are a veteran receiving service-connected care, your copayments are covered by the VA through the Choice Program. Veterans receiving non-service connected care through other health insurance (OHI) are not covered by the Choice Program. In this case, your private health insurance will be the primary payer and you are responsible for making any copayments at the point of service. Please note that Medicare, Medicaid or TriCare plans are not considered OHI under the Choice Program. Veterans experiencing any billing issues can call the VA Community Care Call Center at 1-877-881-7618 (9 a.m. to 5 p.m. Eastern Time) for assistance.

Additional Program Information

  • The Choice Program does not impact your existing VA health care or any other VA benefit.
  • If you did not receive a Choice Card or if you cannot find your card, please call 1-866-606-8198 to learn more about eligibility for the Veterans Choice Program.
  • Care in your community is only covered by VA for medical needs which have been approved by your VA physician.
